Carers If you are a Carer, please let us know – we may be able to help you. We can ensure that your appointments at the Practice fit around your caring responsibilities. We will always offer you a flu vaccine to ensure that you stay fit and well in order to carry out your caring duties. We can offer you a Carer’s Health Check with our Healthcare Assistant, and can refer you to a local Carers’ centre for additional support and advice.

Birmingham Carers Hub recognise that each caring role is unique, and may encompass a range of tasks and responsibilities. They are able to offer help and advice specifically for carers like you, and pride themselves on making a real difference to the lives of people caring for elderly, frail, sick or disabled family members.

Solihull Carers Trust
Solihull Carers Trust is a charity that provides free help and support to unpaid carers of all ages. They provide training and social activites to ensure that no carer ever feels isolated.
Dementia Information & Support for Carers
DISC allow self-referals. They help prevent hospital admissions for dementia patients by doing direct referrals to:
● District nurses (to help with hygiene, incontinence, weight loss, pressure sores, SALT referral)
● Dieticians (to get fortisips etc)
● Occupational therapists ( to get aids in the house)
● Social services
● Emergency respite for carers
● Contact rapid response (to get emergency carers)
